The file is not downloaded with the correct document name.

Sometimes the browser's PDF viewer allows you to download the file you are viewing, but this preview is not the real file, so if you try to download it from the viewer itself, it will probably not be downloaded with the expected name:

The download must always be done from the download links of the application itself, for example inside the document as in the following image:
